Scalp Discoloration from Selenium Sulfide Shampoo: A Case Series and Review of the Literature

Abstract:  We describe six patients who developed orange to red–brown scalp discoloration after application of selenium sulfide–containing shampoos. This phenomenon has been described only once previously. In all of the cases, the discoloration resolved shortly after discontinuing the selenium sulfide. Lightly swabbing with isopropyl alcohol facilitated removal of the discoloration. This simple technique serves as a painless diagnostic method, which may prevent unnecessary evaluation with a biopsy.

Salicin regulates the expression of functional ‘youth gene clusters’ to reflect a more youthful gene expression profile

Synopsis

There are a variety of biological mechanisms that contribute to specific characteristics of ageing skin; for example, the loss of skin structure proteins, increased susceptibility to UV-induced pigmentation and/or loss of hydration. Each of these biological processes is influenced by specific groups of genes. In this research, we have identified groups of genes associated with specific clinical signs of skin ageing and refer to these as functional ‘youth gene clusters’. In this study, quantitative real-time polymerase chain reaction (qPCR) was used to investigate the effects of topical application of salicin ...

Automatic detection of basal cell carcinoma using telangiectasia analysis in dermoscopy skin lesion images

Background: Telangiectasia, dilated blood vessels near the surface of the skin of small, varying diameter, are critical dermoscopy structures used in the detection of basal cell carcinoma (BCC). Distinguishing these vessels from other telangiectasia, that are commonly found in sun-damaged skin, is challenging.

Methods: Image analysis techniques are investigated to find vessels structures in BCC automatically. The primary screen for vessels uses an optimized local color drop technique. A noise filter is developed to eliminate false-positive structures, primarily bubbles, hair, and blotch and ulcer edges. From the telangiectasia mask ...

A Case of Sclerodermatous Graft-versus-Host Disease Responsive to Imatinib Therapy

Abstract:  Sclerodermatous graft-versus-host disease (sGVHD) is a rare, late complication of hematopoietic cell transplantation. Classified as a variant of chronic graft-versus-host disease, sGVHD is thought to be predominantly an immune-mediated response characterized by aberrant T-cell function and dysregulation of tyrosine kinase cascades. Recently, the profibrotic cytokine transforming growth factor B and stimulatory autoantibodies against the platelet-derived growth factor receptor have been implicated in the pathogenesis of sGVHD. Treatment of sGVHD remains disappointing and largely limited by systemic side effects. Imatinib mesylate is a small molecule tyrosine kinase inhibitor that has ...

Satellite Lesions in Congenital Melanocytic Nevi—Time for a Change of Name

Abstract:  The term “satellite lesions” is used in many conditions in dermatology, generally to describe smaller lesions near the edges of a principal lesion. An online medical dictionary gives the definition “a smaller lesion accompanying a main one and situated nearby,” and this can apply both macroscopically and microscopically. The implication is that the smaller lesions have spread from the parent lesion. Given this definition and usual understanding of the term its use is not apt in the case of congenital melanocytic nevi (CMN). In the vast majority of cases ...

Multiple familial and pigmented basal cell carcinomas in early childhood – Bazex–Dupré–Christol syndrome

Abstract

Background  Bazex–Dupré–Christol syndrome (BDCS) is an X-linked dominantly inherited disorder affecting hair follicle structures. Currently, hypotrichosis, follicular atrophoderma and basal cell carcinomas are considered frequent symptoms of the disorder whereas milia are supposed to reflect infrequent clinical signs. Usually, basal cell carcinomas in this disease manifest from the second decade of life onwards.

Case report  Here, we studied a novel multigeneration family of German origin with BDCS. Interestingly, two family members developed pigmented basal cell carcinomas in early childhood, at the age of 3 and 5 years, respectively. The differentiation from other ...

In vivo reflectance confocal microscopy imaging of vitiligo, nevus depigmentosus and nevus anemicus

Background/purpose: Hypopigmentary skin disorders such as vitiligo, nevus depigmentosus and nevus anemicus are common diseases in clinic. The lesions of these diseases could be similar to some extent, although each of them has its own characteristic clinical appearance and histological features. Clinically, the atypical lesions are often difficult to be differentiated. In vivo reflectance confocal microscopy (RCM) is a non-invasive, repetitive imaging tool that provides real-time images at a nearly cellular histological resolution. Our aim was to investigate the RCM features of vitiligo, nevus depigmentosus and nevus anemicus.

Wide spectrum of filaggrin-null mutations in atopic dermatitis highlights differences between Singaporean Chinese and European populations

Abstract

Null mutations in the filaggrin gene (FLG) cause ichthyosis vulgaris (IV) and predispose to atopic dermatitis (AD). Cohort studies in Europe and Japan have reported a FLG mutation carrier frequency between 14 and 56%, but the prevalent European FLG mutations are rare or absent in Chinese patients with IV and AD. To further investigate the spectrum of FLG-null mutations in Chinese patients and compare it with other populations, we conducted comprehensive FLG genetic analysis in a discovery cohort of 92 Singaporean Chinese individuals with IV and/or moderate-to-severe AD. Hereditary tumour syndromes featuring basal cell carcinomas

Abstract

Basal cell carcinoma (BCC) is the most frequent cutaneous malignancy worldwide. This skin tumour is characterised by a broad phenotypic variability and distinct histopathological subtypes. It shows slow, locally invasive growth and only rarely metastasises. BCCs can occur either sporadically or in the context of genetic syndromes, including Gorlin syndrome, Bazex-Dupré-Christol syndrome (BDCS), Rombo syndrome, Oley syndrome, and Xeroderma pigmentosum (XP). To date, different genes and signalling routes have been shown to play an important role in the development and growth of these tumours, including the hedgehog and Wnt/beta-catenin pathway. ...

Viral oncogenesis and its role in nonmelanoma skin cancer

Summary

In recent years, the contribution of viruses to cutaneous oncogenesis has steadily gained recognition. The archetype is human herpesvirus 8, which is well established as the causative agent in Kaposi sarcoma. Other viruses believed to play a role in nonmelanoma skin cancer include human papillomavirus and the recently described Merkel cell polyomavirus. We review the mechanisms by which these three viruses interact with the host cell, ultraviolet radiation and immunosuppression to result in carcinogenesis.

tert-Butyl hydroperoxide, an organic peroxide, causes temporary delay in hair growth in a neonatal rat model

Summary

tert-Butyl hydroperoxide (tBHP), an organic peroxide, has been shown to cause irreversible damage to keratinocytes in vitro with prolonged administration at high concentrations, and reversible damage with short-term administration at low concentrations. To investigate the effects of tBHP on keratinocytes in vivo, we analysed hair growth in tBHP-treated neonatal rats. Sprague–Dawley and Long–Evans rat pups were injected subcutaneously with tBHP or vehicle once daily for 6 days, and hair growth was monitored. The tBHP-treated rats had a significant delay in hair growth. However, this delay reversed within days, and the hair ...

Ability to self-detect malignant melanoma decreases with age

Summary

The prognosis of malignant melanoma depends on the thickness of the tumour. In this study, we analysed the trends in Breslow thickness in 63 patients referred to our institution, a tertiary dermatology referral centre. The mean thickness of melanoma was 0.31 mm, which was lower than the national average of 1.10 mm. There was a significant trend towards increased melanoma thickness with increasing age, with a rate of 0.24 mm (95% CI 0.12–0.37) for each additional 10 years of age above the age of 20 years. This trend was only apparent in cases of self-diagnosed ...

Sunnier European countries have lower melanoma mortality

Summary

Doubt has been cast on sunlight as the major causative factor for malignant melanoma. We performed statistical analysis of the average annual sunlight hours in 36 European capital cities compared with the country’s melanoma mortality rate. A significant inverse proportionality was identified in both men and women, indicating that sun exposure is unlikely to be the strongest factor affecting mortality from malignant melanoma.

The effect of ageing on phenotype and function of monocyte derived Langerhans’ cells

Summary

Background  With increasing age the immune system shows functional decline. In the skin this is associated with an increased incidence of epidermal malignancies and infections. Epidermal Langerhans’ cells (LC) act as sentinels of the immune system recognising, processing and presenting antigen and inducing T cell responses. Previous investigations have demonstrated a reduction in the number of epidermal LC in elderly subjects. Moreover, the ability of LC to migrate in response to tumour necrosis factor (TNF)-α, but not interleukin (IL)-1β, is significantly impaired in the elderly.

Contagious Itch in Humans. A Study of Visual “Transmission” of Itch in Atopic Dermatitis and Healthy Subjects.

Abstract.

Anecdotal evidence suggests “contagious” itch occurs in daily life when we see other people itch and scratch. This phenomenon has not been systematically studied previously, and factors which can amplify itch perception were unknown. We investigated whether exposure to visual cues of itch can induce or intensify itch in healthy and atopic dermatitis subjects. Participants received histamine or a saline control delivered to the forearm and were asked to watch short video clips of people scratching. Spontaneous scratching induced by visual cues was monitored and analyzed. Atopic dermatitis patients reported ...
